Caleb Graham
Caleb Graham, MS, CPHIMS is a Clinical Informatics expert with a focus on Nephrology and Value- Based Care. He currently leads the informatics program at Dallas Nephrology Associates, one of the largest nephrology practices in the U.S., where he successfully implemented the Epic EHR system. Prior to this, he served at Baylor Scott and White Health, the largest not-for-profit healthcare system in Texas, where he implemented and optimized both the AllScripts Sunrise and Epic Electronic Health Record systems. Caleb's work spans all medical specialties and diverse care settings, both domestically and internationally. He holds a Master of Science in Health Informatics Administration and is a Certified Professional in Health Information and Management Systems and a Certified Health Informatics Administrator.
